  • Claimant was injured when slipping and falling while carrying steel rods. Claimant underwent four surgeries to the right shoulder and received multiple injections. Claimant was unable to return to pre-injury employment. Claimant was also diagnosed with depression related to the claim and inability to work.

 

  • Injury:              Ulnar deviation of three fingers and ulnar neuropathy

  • Claimant was injured after sustaining a crush injury to the hand that required surgical repair. Claimant underwent conservative treatment following surgery.

  • Claimant was injured after slipping and falling on ice. Claimant received one injection to the right shoulder.

  • Claimant sustained injury to back after prolonged standing at work. Claimant received conservative treatment that included physical therapy and returned to pre-injury employment.

  • Claimant injured the left knee after striking it against a metal table. Claimant was treated conservatively with therapy. As a result of the injury to the left knee, Claimant began having pain with his right knee due to overcompensation. The right knee was also treated conservatively with therapy.

  • Claimant was injured after falling at work injuring the shoulder and ankle. Claimant underwent two surgeries to the shoulder. Claimant was able to return to pre-injury employment.

  • Claimant was injured while picking up heavy objects at work. Claimant underwent a two-level fusion in the lumbar spine. Claimant was unable to return to pre-injury employment, but was able to return to work with restrictions.

  • Claimant was injured after slipping and falling at work. Claimant underwent a four-level fusion to the spine. Claimant was unable to return to pre-injury employment.

  • Claimant was injured after slipping and falling at work. Claimant was treated with physical therapy and was able to return to pre-injury employment.

  • Claimant was injured after lifting heavy objects at work. Claimant had injections to the low back and physical therapy. Claimant was able to return to pre-injury employment.

  • Claimant developed an inguinal hernia requiring surgery after pushing motorcycles out to be displayed from a garage.  Following surgery, injured worker underwent a course of physical therapy before being discharged to return to work without restriction.

  • Amount: $16,000
  • Claimant was assaulted by coworkers during the course of his employment.  He underwent a course of physical therapy and wore an ankle brace during his recovery. He missed approximately 2 months of work before being cleared to return.

  • Claimant injured his back lifting barrels of clothing during the course of his employment.  His treatment included a course of physical therapy.

 

  • Injury: L5-S1 disc herniation
  • Body Part: back

  • Claimant injured his back carrying large amounts of food during the course of his employment.

 

  • Injury: Contusion

  • Claimant’s contusion did not require any treatment, but was on light duty for two weeks.

 

  • Injury:  Lesion of the ulnar nerve
  • Body Part: Elbow
  • Amount: $ 26,381.25
  • Claimant sustained injury to elbow during the course of employment as a nurse.  Injured workers underwent surgery; specifically, anterior subcutaneous ulnar nerve transposition.  Claimant worked an extensive period of light duty before being released without restriction approximately one year after her date of injury.

 

  • Injury:  Acromioclavicular joint dysfunction & rotator cuff impingement
  • Body Part: shoulder
  • Amount: $ 25,000
  • Claimant injured the right shoulder while hanging wet clothes on a rack to dry.

 

  • Injury:  Sprain
  • Body Part: back & neck
  • Amount: $45,000
  • Plaintiff was involved in a motor vehicle crash. Vehicle was struck by another vehicle who failed to yield the right of way. Treatment included physical therapy, massage therapy, chiropractic treatment, and trigger point steroid injections.

 

  • Body part:  Forearm
  • Amount:  $2,016
  • Worker was burned on the forearm by hot grease.  Treatment consisted of topical cream and pain medication.

 

  • Body part:  Knee
  • Amount:  $10,080
  • Claimant was injured when he fell while climbing out of a truck.  An MRI showed a medial meniscus tear which was operated on arthroscopically and then treated with physical therapy.

 

  • Body part:  Knee
  • Amount:  $15,435
  • Police officer was injured while making an arrest.  An MRI showed a medial meniscus tear which was surgically repaired and treated further with physical therapy and pain medication.

 

  • Body parts:  Neck and Back
  • Amount:  $21,775
  • Public safety worked slipped and fell on ice, landing on his neck and back. Disc bulges were found.  Treatment consisted of chiropractic care.

 

  • Vocational Rehabilitation.
  • Amount $5,500
  • Claimant’s job was unable to accommodate light duty restrictions.  Rather than enter into vocational rehabilitation, the injured worker accepted a settlement in lieu of those services.

  • Amount:  $14,500
  • Injured worker slipped and fell in a parking lot.  X-rays revealed non-displaced fractures in the arm which were treated with a cast, splint, and home exercises.

 

  • Body part:  Back
  • Amount:  $10,020
  • Worker was injured while lifting and stocking heavy boxes.  Treatment consisted of physical therapy, epidural injections, and pain medication.

  • Worker was burned on the face and hands by molten plastic.  A skin graft and several moths of physical therapy were required.
