  • Claimant was diagnosed with carbon monoxide poisoning after being exposed to fumes from a faulty machine at work. Claimant was seen at the emergency room and had one follow up visit with the doctor.

  • Claimant was injured after tripping and falling. Claimant underwent conservative treatment that included physical therapy.

  • Claimant was operating a forklift and got his foot stuck between two palletjacks, which caused a deep laceration of the right second toe. He underwent surgery to remove a portion of the toe.  This was followed by a course of physical therapy.  Claimant returned to work without restriction.

  • Claimant was working at any auto repair shop when stuck with a large object. Treatment included a course of physical therapy, one cortisone injection to the shoulder, and one epidural injection to the cervical spine.

 

  • Injury: Multi-level disc herniation
  • Body Part: Cervical spine
  • Amount: $42,000
  • Claimant was injured moving heavy furniture in the course of employment. Claimant initially had a pain management injection before subsequently undergoing a multi-level fusion surgery.  The Claimant missed several months from work, since returning to pre-injury employment.

 

  • Injury: Complex tear of the medial meniscus posterior horn and body
  • Body Part: Knee

  • Claimant twisted knee chasing a child during the course of employment as a teacher. An MRI revealed a meniscus tear.  Claimant did not undergo surgery, but treated with with a course of physical therapy.

 

  • Injury: Orbital blowout fracture
  • Body Part: Face, eye
  • Amount: $35,000
  • Claimant was struck in the face by a stapler during the course of his employment as a teacher. He underwent surgery for a right orbital blowout fracture, which was repaired with a Medpor titanium implant.  He did not have any other treatment following surgery besides occasional follow up appointments.  Claimant missed approximately 7 months from work before returning to his preinjury employment.

 

  • Injury: Distal tibia/fibula fracture
  • Body Part: Leg
  • Amount: $30,150
  • Claimant tripped on a cord on the floor and fractured her right ankle. She was placed in a walking boot and treated nonoperatively.  She underwent a course of physical therapy after the boot was removed.  She also went an FCE which placed Claimant on permanent work restrictions.  She remained with the same employer, who transferred her to a new light duty position.

  • Client was involved in a motor vehicle collision.  Injuries were sustained to the neck, back and shoulder.  Treatment included medication and chiropractic care.

  • Claimant was injured at work.  Diagnosis included a rotator cuff tear.  After an extended course of therapy, the injured worker underwent arthroscopic surgery followed by further physical therapy.
