  • Claimant sustained injury to the ankle after it rolled while moving an exam table at work. Claimant was treated with bracing and therapy. Claimant was able to return to pre-injury employment.

  • Claimant was injured after lifting heavy objects at work. Claimant reopened claim for worsening of condition and underwent additional physical therapy, ultimately returning to pre-injury employment.

  • Amount: $4,185.00
  • Claimant sustained injury after lifting the hood of a car while at work. Claimant was treated conservatively with therapy and returned to pre-injury employment.

 

  • Injury: Distal fibular and posterior malleolar fracture & Achilles injury

  • Claimant was injured after falling at work. Claimant underwent multiple surgical procedures, involving the ankle and the leg, and then subsequent therapy.

  • Claimant was injured after slipping and falling in water, striking the head on the ground. Claimant was treated with medication, rest, and doctor’s visits for monitoring condition. Claimant returned to pre-injury employment.

 

  • Injury: Commuted impacted distal radius & ulnar styloid process fractures
  • Body Part: Right wrist

  • Claimant was injured when loading a load on a trailer and fell from trailer directly on the wrist. Claimant underwent surgery to the wrist. Claimant returned to pre-injury employment.

  • Claimant injured her hand/wrist lifting a heavy patient. Diagnostics were unremarkable.  Claimant underwent a course of physical therapy before returning to work.

  • Claimant injured twisted his knee handling a jackhammer in the course of employment. He underwent an MRI which was negative.  He had about two months of active care which included cortisone injections but not physical therapy.

 

  • Injury: Disfigurement
  • Body Part: Foot
  • Amount: $1,500
  • Claimant burned her foot when boiling water was spilled on her. She subsequently developed a scar and was compensated for the disfigurement.

 

  • Injury: Non-displaced fracture
  • Body Part: Elbow

  • Claimant slipped and fell at work and fractured her elbow. She had approximately 4-5 weeks of physical therapy and then returned to work 2 months after the date of injury.

  • Claimant slipped in an icy parking lot during the course of employment. An MRI was did not show any tearing or anything else requiring surgery.  Treatment included approximately 7 weeks of physical therapy. Claimant did not miss any time from work.

 

  • Injury: Sprain, pulmonary embolism
  • Body Part: Ankle, Lungs
  • Amount: In excess of $25,000.00
  • Claimant sprained ankle at work and was placed in a walking boot. Due to the boot immobilization he developed a pulmonary embolism in his lung, which required medication.

 

  • Injury: Strain
  • Body Part: Back
  • Amount: $4,300
  • Claimant injured her back lifting heavy objects. Diagnostic imaging was negative.  Injured worker underwent a course of physical therapy.

 

  • Injury: Laceration
  • Body Part: Hand
  • Amount: $1,670
  • Claimant cut hand during the course of employment as a delivery driver. The injury required stitches, and driver was compensated for the scar on the palm.

  • Amount:   More than $15,000.00
  • Public safety employee was injured as a result of stepping in a hole and twisting knee.  Diagnosis included medial lateral and meniscal tears which required surgical repair.

 

  • Body part:   Elbow
  • Amount:   $5,025
  • Public safety employee stuck his elbow against a metal door while making an arrest.  Diagnosis was a chip fracture of the elbow.

  • Claimant was injured when slipping and falling while unloading a truck.  Diagnosis included sprains, which were treated with chiropractic care.

  • Amount:   $12,500
  • Claimant was injured when struck by falling containers of soup.  Diagnosis included shoulder tendinopathy, which was treated with an injection.

 

  • Body parts: Shoulder and knee
  • Amount:   $20,000
  • Injury occurred when worker was struck in the back of the head by a falling item.  Diagnosis included shoulder tendinopathy and a knee contusion.  Treatment included a knee brace and chiropractic care.

 

  • Body part:   Index finger
  • Amount:   $4,020
  • Claimant struck his finger on a shelf bracket. Surgery was required.

 

  • Body part:   Knee
  • Amount:   $38,000
  • While loading tools into a truck, claimant’s knee struck a step and was hyperextended.  An MRI revealed a tear of the medial meniscus.  Treatment included physical therapy, injections, and an arthroscopic surgery.
