  • Injury: L3-4 disc bulge with annular tear

  • Claimant was injured after lifting heavy objects at work. Claimant underwent a laminectomy procedure and was able to return to work.

 

  • Injury: Non-displaced radial head fracture. TFCC tear

  • Claimant injured shoulder while unloading product at a hardware store. Claimant underwent an MRI that revealed a possibly pinhole tear in the labrum. Arthroscopic surgery was performed including debridement of the rotator cuff and labrum.   Claimant then had post-op physical therapy and one cortisone injection.

  • Claimant injured back while moving equipment at work. An MRI scan of the back revealed a bulging disk in the lumbar spine. The necessary treatment included chiropractic sessions and an epidural steroid injection.

  • Claimant sustained a severely lacerated index finger while working. The Claimant underwent a partial amputation above the knuckle and returned to work in a matter of weeks.

  • Claimant injured shoulder while digging trees from the ground. The injury required arthroscopic surgery including labral debridement, subacromial decompression, and open excision of the distal clavicle.  Claimant also received an injection and underwent a course of physical therapy.

  • Claimant strained his back while lifting at work and underwent a course of physical therapy.

  • Claimant sustains burns to face and body when he was hit with hot tar. He underwent surgical intervention on the burn sites which was followed by prolonged post operative care including medications and wound care.

  • Injury: Fracture, carpal tunnel syndrome, cubital tunnel syndrome

  • Claimant’s arm was crushed by a large pipe. Surgery was performed to repair a distal radius fracture.  Postoperatively the Claimant developed carpal tunnel syndrome and cubital syndrome which required a second operation.  This was followed by a course of therapy.

  • Claimant injured neck and shoulder as a result of a whiplash injury. The necessary treatment included 8 sessions of physical therapy.

  • Claimant sustained a head and neck injury during a motor vehicle accident. Diagnostic imaging was negative, but the Claimant suffered from post traumatic headaches, requiring medication.  For the neck strain chiropractic sessions were prescribed.

  • Claimant’s head struck a brick wall when a floor cleaner malfunctioned and kicked back into the wall. As a result of the trauma the Claimant suffered from headaches and blurry vision.

  • Claimant’s thumb was slammed in a heavy door at work. Xrays revealed a fracture and the thumb was splinted for a period of time.  Once the fracture healed the Claimant underwent a course of hand therapy.

  • Claimant sustained injury lifting a heavy bag of coins. An MRI did not reveal any tears. Treatment included physical therapy and two injections.

  • Claimant’s foot was ran over by a cart at work. An xray revealed a fracture of the medial malleolus.   Surgery was performed in the form of an ankle arthroscopy with removal of multiple loose bodies.  Claimant underwent a course of post operative physical therapy before being released.

  • Claimant injured back while lifting product at work. An MRI revealed a lumbar disk herniation.  Treatment included one epidural steroid injection and a course of work hardening.
