She had a MRI performed which revealed a bone bruise at the greater tuberosity and thickening of the suprapinatus and infraspinatus tendons, a small focal tear at the proximal spraspinatus tendon. She was also given an injection and had extensive course of therapy.

  • Claimant was injured at work when a bay door fell on his left arm. He required surgery and physical therapy but was able to return to work full duty.

  • Claimant injured his upper back and neck at work when he was unloading a pallet. He was evaluated at a medical center and had x-rays of his cervical and thoracic spine.  He had physical therapy.

  • Claimant was injured at work pulling a heavy cart. He was evaluated at a medical center where x-rays were performed and was placed in a knee immobilizer.  He was referred to an orthopedic specialist.  An MRI was done which showed a tibial plateau fracture.  He was given a knee brace and required periodic x-rays to monitor healing until the fracture.

  • Claimant worked for a period of time in a temporary tent while his employer’s showroom was being remodeled. Eventually he noted onset of cough and other breathing issues due to suspected conditions inside the tent. He was treated with medication.

  • Body Part: Back, Right Hip
  • Amount: $118,800.00
  • Claimant injured his back and right hip at work when he tripped and fell on a strap while moving pallets off of a trailer. He was taken by ambulance to hospital where he was diagnosed with a right hip fracture.  He was admitted and underwent a total right hip replacement surgery.  He was discharged to an acute rehab facility where he stayed for 5-6 days and was then discharged home.  He continued with physical therapy.  He also had an MRI of the lumbar spine which lead to a series of epidural steroid injections.

  • Claimant was injured at work when she was holding a door on a very windy day when a gust of wind blew back the door causing her to fall and twist her right ankle. She was evaluated at St. Joseph’s Medical Center where x-rays showed a nondisplaced fracture of the distal fibula.  She was given a splint and crutches.  She was evaluated by Dr. Stern who placed her in an air splint. She had a course of physical therapy and was released.

  • Claimant injured his back while lifting at work. He had physical therapy and one lumbar injection.  He was released back to full duty.

  • Claimant was injured at work when he fell onto his right leg. He underwent a repair of the right quad tendon disruption through a transosseous patellar tunnel.  His knee was immobilized with a hinge brace which was adjusted by increments.   He had a fall during his aquatic therapy resulting in a rerupture of the quad tendon.  He underwent another surgical repair of the right distal quad and had additional therapies.

  • Plaintiff was the belted driver of his vehicle when he was struck on the passenger side by a commercial truck that had drifted into plaintiff’s lane of travel, after both vehicles were stopped at a red light and began making left hand turns onto an adjacent roadway. The plaintiff’s injuries required two month of therapy with no loss of time from work.
