After a period of vocational services, the Claimant returned to work earning higher wages.

- Claimant sustained an injury to the knee and hip when they fell over a box while working in a warehouse. Claimant underwent physical therapy, medications, and an EMG. Claimant was ultimately released to full duty at the same employer but opted for different employment.

- Claimant sustained an injury to the ankle when they stepped off of the back of a work truck. Claimant underwent an X-ray which showed an oblique fracture of the distal left fibula. Claimant was placed in a boot and had physical therapy. The supplemental x-ray showed the fracture had healed, and claimant was able to return to their pre-injury employment on a full duty basis.

- Claimant sustained a de-gloving injury to the foot when it was pinned between a forklift and a wall. Claimant underwent several surgeries, including skin grafts. Claimant underwent substantial therapy, medications, and a subsequent debridement and irrigation to repair the dorsalis artery. Claimant was ultimately released to working additional duties with a different employer.
