  • Claimant slipped on ice and twisted the ankle walking into work. Claimant reopened claim for additional physical therapy.

  • While chasing a fleeing suspect, claimant fell and injured both knees. Claimant had surgery on one knee and an injection to the other knee. Claimant was able to continue pre-injury employment.

  • Claimant injured wrist while drawing blood. Claimant underwent surgery for de Quervain’s and therapy following surgery.

  • Claimant was involved in a motor vehicle collision while working. Claimant had two surgeries to each shoulder and a fusion surgery to the back. Claimant had treatment for a concussion and depression. Claimant was released from care with permanent work restrictions and had to change jobs.

  • Client was working in a wood shop and sustained an injury lifting and moving pallets. The worker underwent a nerve conduction study, which did not reveal the need for surgery.  Claimant underwent physical therapy and was release from care.

  • Worker sustained a head and neck injury as a result of driving go-karts during a “team building” event off-site at a go-kart track. Diagnosis was on a concussion and post-traumatic headaches.  Claimant underwent neurological care and physical therapy for the neck.

  • Worker stepped into a pothole on fell, sustained injuries to the head, face, left elbow, left arm, right wrist, back, right knee and left ankle. No fractures and Claimant underwent physical therapy.  After completing the course of care, post-traumatic headaches continued.   Employee lost no time from work and continued to work for the Employer in the same position.

  • Worker’s foot was caught in a raised piece of carpeting and fell, sustaining injury to both hips.

  • Claimant sustained injury when using a spanner wrench on a stuck bolt. While pushing upward, the worker felt an instant pain in the neck and low back.  Claimant underwent cervical and lumbar transforaminal epidural steroid injections.

  • Claimant sustained injury to the low back while moving cases of soda and building in-store displays. Worker came under the care of an orthopedist, who recommended an MRI.  The MRI showed a broad based disc protrusion at L5-S1 with no root cord compromise.  Claimant was evaluated by a neurosurgeon, who did not recommend surgery, but did recommend lumbar epidural steroid injections, which the worker declined.  After a period of vocational services, the Claimant returned to work earning higher wages.

  • Claimant injured the wrist while working as a server in a restaurant. Sustained a non-displaced fracture that extended over the base of the radial styloid.  Worker also strained the volar and dorsal extrinsic ligaments and the scapholunate ligament.  Non-surgical.  Client returned to work in a new position with a new employer.  Part-time employment.

  • Claimant was at work and fell from a ladder sustaining injuries to the neck, back and knee. Claimant was treated with pain medication, physical therapy, and injections.

  • Claimant was working as a physical therapist helping a patient from a tub to a wheelchair when they injured their neck, shoulder and back. Claimant was treated with a sling and physical therapy. An MRI of the shoulder was performed and revealed a small tear in the supraspinatus tendon.

  • Claimant fell stairs while carrying a bucket sustaining injury to the head, shoulder, back and neck. Claimant was treated with physical therapy and injections.  An MRI of the shoulder was performed which showed a small full thickness tear of the supraspinatus tendon.  Claimant underwent an arthroscopic surgery to repair the rotator cuff.

  • Claimant was at work when their hand was caught in a roller conveyor. Claimant underwent an irrigation and debridement procedure, which was followed by a course of physical therapy.

  • Claimant was working as a transmission technician when they pulled a pallet sustaining injury to the shoulder. Claimant was treated with medication, physical therapy, injections, and work hardening. An MRI of the shoulder was preformed and showed a partial thickness tear of the supraspinatus tendon. Claimant underwent surgery in the form of an arthroscopic rotator cuff repaid with subacromial decompression.

 

  • Injury: Rotator Cuff Tear
  • Body Part: Shoulder

  • Claimant slipped and fell on concrete steps sustaining injury to the shoulder. Claimant was treated with a sling, medication, physical therapy.  An MRI of the shoulder was performed and showed a full thickness tear with retraction of the distal supraspinatus tendon.  Claimant underwent surgery of the shoulder in the form of an arthroscopic rotator cuff repair, biceps tenotomy, subacromial decompression and debridement.

 

  • Injury: Acute Thoracic Spine Strain
  • Amount: $5,160.00
  • Claimant sustained an injury to the thoracic spine after suffering direct trauma to the area. Claimant underwent a course of physical therapy and was able to return to full-duty employment at the same employer.

 

  • Injury: L4-L5 Disc Protrusion
  • Amount: $10,908.00
  • Claimant sustained an injury to the lumbar spine when lifting heavy luggage. Claimant underwent an MRI which revealed a disc protrusion at L4-L5.  Claimant received physical therapy, medications, and one steroid injection, prior to being released to full-duty at the same employer.

 

  • Injury: Traumatic injury to the knee and hip
  • Amount: $11,000
  • Claimant sustained an injury to the knee and hip when they fell over a box while working in a warehouse. Claimant underwent physical therapy, medications, and an EMG.  Claimant was ultimately released to full duty at the same employer but opted for different employment.

 

  • Injury: Strain, Sprain, Contusion to the back, neck, hand, and ankle.
  • Amount: $8,000 (in addition to the $7,947.50 already awarded)

  • Claimant was injured when she slipped on ice while at work. Claimant underwent conservative care in the form of physical therapy and was released to similar employer with a different company.

 

  • Injury: Traumatic injury to the ankle
  • Amount: $7,000
  • Claimant sustained an injury to the ankle when they stepped off of the back of a work truck. Claimant underwent an X-ray which showed an oblique fracture of the distal left fibula.  Claimant was placed in a boot and had physical therapy.  The supplemental x-ray showed the fracture had healed, and claimant was able to return to their pre-injury employment on a full duty basis.

 

  • Injury: Degloving of the foot
  • Amount: $123,279.68
  • Claimant sustained a de-gloving injury to the foot when it was pinned between a forklift and a wall. Claimant underwent several surgeries, including skin grafts.  Claimant underwent substantial therapy, medications, and a subsequent debridement and irrigation to repair the dorsalis artery. Claimant was ultimately released to working additional duties with a different employer.
