  • Injury: Rupture of the ulnar collateral ligament
  • Body Part: Left Thumb
  • Amount: $3,944.50
  • Claimant was injured after a patient grabbed the thumb causing injury. Claimant underwent surgical repair to the ruptured ligament and post-operative therapy.

 

  • Injury: Thoracic and lumbar strain
  • Body Part: Back
  • Amount: $9,860.00
  • Claimant was injured after lifting a heavy patient at work. Claimant was treated conservatively with medication and therapy.

 

  • Injury: Lateral patellar dislocation

  • Claimant was injured after twisting and falling at work. Claimant had two surgeries. The claim was reopened for additional physical therapy.

  • Due to stresses of being a law enforcement officer, Claimant developed hypertension. Claimant manages the hypertension with medication.

  • Injury: Quadriceps tendon rupture
  • Amount: $2,976.00
  • Claimant was a day-laborer who was hired for a one-day job unloading a large truck. The Claimant was injured after tripping on the ramp of the truck.  Claimant was diagnosed with a quadriceps tendon rupture and underwent surgery.  Post-operatively the Claimant underwent a course of physical therapy.

  • Injury: Ankle tendinitis
  • Amount: $5,000.00
  • Claimant’s ankle was injured when a co-worker dropped a box onto the Claimant’s foot. Xrays did not reveal a fracture and an MRI was unremarkable. The Claimant was initially non-weight bearing on crutches prior to undergoing a course of physical therapy.

 

  • Body Part: Neck, scalp
  • Injury: Disfigurement to the scalp, cervical spine burst fracture
  • Amount: $61,911.50
  • Claimant was involved in a motor vehicle crash where the tractor trailer rolled onto the passenger side panel causing injury to the neck. A CT scan of the cervical spine revealed a burst fracture with retropulsion of the fracture fragments.  The Claimant underwent an anterior C7 corpectomy and C6 to T1 fusion with cage allograft and anterior cervical fusion.  Post-operatively the Claimant was in a neck brace for a period of time but eventually was released to return to work.

  • Injury: Sprain, post traumatic headaches
  • Amount: $14,625.00
  • Claimant sustained injury after tripping and falling at work. Treatment included rest and physical therapy. An MRI revealed a disc protrustion in the cervical spine.

  • Injury: Sprain/strain leading to adhesive capsulitis
  • Amount: $12,500.00
  • Claimant was working as a nurse and sustained injury lifting a patient. An MRI revealed rotator cuff tendinosis and a course of physical therapy was initiated.  During a prolonged recovery the Claimant developed adhesive capsulitis that required one cortisone injection.

 

  • Injury: L4/5 disc protrusion
  • Body Part: Back
  • Amount: $6,880.00
  • Claimant was at work as a registered nurse while pulling a patient out of bed sustained an injury to the back. An MRI of the lumbar spine revealed a minimal disc bulge with some superimposed mild broad-based paracentral posterior disc protrusion at the L4-5 level.  An MRI of the thoracic spine was negative for herniated discs and an EMG/nerve conduction study was negative for lumbar radiculopathy.  The required treatment was a course of physical therapy which was followed by a full duty work release.

 

  • Injury: Partial amputation
  • Body Part: Index finger, thumb
  • Amount: $41,242.50
  • Claimant’s thumb and index finger were crushed in a printing press. The injury required two surgeries in the form of a partial amputation and skin graft from the forearm.  This was followed by a course of occupational therapy and an eventual full duty work release.

 

  • Body Part: Back, hip
  • Injury: Lumbar strain
  • Amount: $8,600.00
  • Claimant fell on a loading dock injuring the hip and back. The necessary treatment included physical therapy and one injection in the hip.

 

  • Body Part: Head
  • Injury: Concussion
  • Amount: $11,050.00

Claimant was hit in the head by a steel pole at work and suffered a concussion.  This resulted in blurry vision and headaches.  The Claimant treated with medication and was instructed to wear sunglasses in bright environments.
